This form screens licensed pharmacists who may participate in a randomized controlled pilot study on pharmacy calculation proficiency (AI-powered tutor compared with traditional study methods). All items must be answered truthfully.

This research study, conducted by the College of Pharmacy and Chemistry at the University of the Immaculate Conception (Davao City), aims to compare the effectiveness of an AI-powered pharmacy calculation tutor versus a traditional self-study handout in improving pharmacy calculation proficiency among licensed pharmacists.
If you are eligible and accepted, you will be randomly assigned to one of two groups and complete a single online session lasting approximately 90 minutes. The session includes a pre-test, an intervention period (either AI tutor or self-study handout), a post-test, and a brief satisfaction survey.
Your participation is entirely voluntary. You may withdraw at any time without penalty. All data will be kept strictly confidential in compliance with the Data Privacy Act of 2012 (RA 10173). Results will be reported in aggregate form only — individual participants will never be identifiable. Full informed consent will be obtained at the start of your study session.
This study has been conducted in accordance with the ethical standards of the UIC Research Ethics Committee (UIC-REC), the Philippine National Health Research System (PNHRS) guidelines, and the Declaration of Helsinki.
